Office Location:

Eastlake Cardiovascular, P.C. is conveniently located at 24211 Little Mack Avenue, just north of Nine Mile Road, in St. Clair Shores.

Appointments:

Please call our office during normal business hours to schedule regular appointments, renew your prescription, or ask routine questions regarding your treatment. Please have your insurance information available when you call to schedule an appointment. Our telephone lines are open weekdays from 8:00 am to noon and 1:00 pm to 5:00 pm. In case of an emergency after business hours, please call our answering service at 586-498-0440 and they will call the physician on duty. One of our physicians is on duty 24 hours a day, including weekends and holidays.

Your First Visit To Our Office:

On your first visit to our office, we ask you to bring your medical records, insurance cards, and any medication you are taking. Prior to your visit we will mail you a new patient registration and medical history form. We ask that you bring these completed forms with you to your first office visit.

Payments:

We accept many types of health insurance. Our helpful staff will tell you if we participate in your particular health plan. We also accept VISA, MasterCard, and Discover. We ask that you pay the amount not covered by your insurance at the end of your appointment.

Cancellations:

If you cannot keep an appointment, please call us at least 24 hours in advance so we can give your time to another patient.

Refilling your Prescriptions:

When calling the office to refill your prescription, please have the phone number of your pharmacy available. Please allow 72 hours for your prescription to be filled.
